How do I request my health records?
Clients may review their records in the presence of a counselor. The request and the fact that a review occurred will be entered in the client's record. Step One: Complete a written request
Required Information:
- Full name, date of birth
- Dates of service
- Type of records requested
Step Two: Submit your written request You may submit your completed written request utilizing the following methods:

Step Three: Attend an appointment An appointment will be scheduled to review your needs and the required documentation. Processing Time: Requests are normally completed within 2 weeks and will be processed no later than 30 days from the written request.
